Client flowchart


We know that there is a bit of confusion around the different services Crew provides, so here is a flow chart of how our clients move through the Support Services dept:

 

Clients refer themselves, by an agency or via our shop

Client brought to referral meeting - Wednesdays @ 10.00am

At referral meeting new referrals are looked at, cases are allocated and clients are offered an assessment date within 2 weeks

At assessment, clients complete an assessment form and confidentiality form with consent for Crew database and SMR (Scottish Misuse of Drugs) database and CORE first assessment form (evaluation tool)

Liaison with other agencies with client's consent e.g. Social Work, Psychiatry

Offer clients person centred waiting time support (20 minute goal orientated one to one support sessions fortnightly) if there is a waiting list and also information on groups, Reiki, ear acupuncture, relaxation etc

Client placed on waiting list

When space arises client is allocated to a practitioner for drug counselling and/or Recovery Development work for groups, one to one relaxation etc

Person centred counselling with practitioner starts, in general meeting every week for one hour sessions, usually meeting fortnightly in ending stage of counselling

Regular reviews take place between practitioner and clients - we can work together for initially 20 sessions which can be increased to 40 sessions for longer term work

Before, during and after counselling, clients can engage with Recovery Development Worker regarding issues such as employment, housing, benefits, education etc

3 months after counselling has ended, clients are offered a follow up session with our Recovery Development Worker
